Download and print the Program Registration Application. (See link on left navigation menu.)

Fill out this form completely. Be sure to sign the Liability Waiver (application will not be processed unless signed).

Fax the registration application with payment information to (949) 724-6608.
Please print legibly and be sure to include your credit card number and expiration date on the application.
Your registration will be processed in order received. If you have not received your receipt within one week, or you have questions, comments, or suggestions, please contact Program Registration at 949-724-6610 or by e-mail at QuickReg@ci.irvine.ca.us.
