What Documents Will Be Required?

1. APPLICATION FORM
2. 500-FOOT BUFFER ZONE MAP. Map must identify the work site and all occupied residences. Measure the distance from the property line of the work site to the property line of the nearest occupied residence, and include the distance on the map.
