Passport Application Types & Requirements

New Applicants

All new applicants must appear in person and complete a DS-11 form.

Supplemental Requirements:

  • Proof of U.S. Citizenship (Original only; no photocopies):
    • Certified birth certificate
    • Previous U.S. passport (if applicable)
    • Certificate of Naturalization or Citizenship
    • Report of Birth Abroad
    • CA Applicants: Must submit a state-certified Certificate of Live Birth (abstracts/hospital records not accepted)
  • Proof of Identity (with photo & signature):
    • Valid driver's license
    • Government-issued ID
    • Recent Certificate of Naturalization/Citizenship
    • If no ID, bring an identifying witness and complete DS-71
  • One passport photo
    • Available at City Clerk’s Office, Office of Records (1st Floor, City Hall) for $15.00 (cash, check, credit, or debit card)
  • Payment:
    • Application Fee (Check/money order to U.S. Department of State)
      • Ages 16 and older: $130.00
      • Ages 16 and under: $100.00
    • Acceptance Fee: $35.00
  • Parental Consent for Minors:
    • Ages 16 and under: Consent from both parents/legal guardians
    • Ages 16-17: Applicant must appear with one parent/guardian

Lost Passports, Stolen Passports, Corrections, and Name Changes

If your valid passport is lost or stolen, or if you need corrections and name changes, you must go to travel.state.gov.

Travel Requirements

  • Under the Western Hemisphere Travel Initiative (WHTI), U.S. citizens re-entering the U.S. by land or sea from Canada, Mexico, Bermuda, or the Caribbean must present one of the following:
    • Passport book
    • Passport card
    • Other DHS-approved travel documents
  • International air travel requires a passport book.
