THE CITY OF IRVINE SHALL HAVE ALL RIGHTS, TITLE AND INTEREST TO THE IMAGE(S) AND THE COPYRIGHTS THERETO, AND MAY USE THE LIKENESS AND/OR PHOTOGRAPH, AS APPLICABLE, OR ANY REPRODUCTION THEREOF, IN ANY FORM OR MEDIUM AS IT MAY SELECT.

Such information is being requested and will be utilized by the City for the specific and limited purpose of future City correspondence regarding the subject-matter of this form.
